Please provide any related information to your symptoms in as much detail as possible so that doctor can better understand and evaluate your lifestyle or any of your habits that may be related to your medical problem(s). Doctor will then provide you with suggestions and recommendations for changes to any habit that may be contributing to your medical problems(s) based on the information you provided to help you live healthier and with a better quality of life for the future. For other medical problem(s): Do your best to prepare a detailed summary of your health condition from the time of your last treatment to your current appointment.
